Candlewood Valley Health and Rehabilitation Center has been providing award winning care in the Litchfield County area for more than 40 years. We offer a broad range of health care services, including, but not limited to: Post Acute Care, Short Term Inpatient/Outpatient Rehabilitation and Long Term Skilled Nursing. Candlewood also has a 42 bed secure unit dedicated to supporting those affected by Alzheimer’s disease and other cognitive conditions. We have established caregiving neighborhoods to facilitate person centered care, where the residents receive care from the same trusted, familiar faces every day.
